Eligibility

Any infant between the ages of 15 days and 12 months who resides permanently in Canada during the protection period and whose name appears on the Peek-a-Boo Plan application submitted to the head office of Industrial Alliance Insurance and Financial Services Inc.

Extent of Protection

The insurance coverage takes effect on the later of the following dates: the 15th day of life of the child or the date of receipt of the application form at the Company’s head office. The protection period is 12 months. The insured infant is covered 24 hours a day, worldwide.

Exclusions

No benefits are payable for:
A.Hospital and paramedical expenses reimbursable by any other private or government insurance plan. In the case of a person who is not covered by a government health plan providing illness or injury benefits, the Company will reimburse only that portion of expenses which would have been reimbursed to a person covered by such a government health plan.

B.Care or treatment given by a person who is related to the insured (except for transportation expenses).



Eligibility

Any child of a same family aged between 2 and 5 years inclusively for whom no diagnosis has been made for a critical illness on the date of enrolment in the program. To be eligible, the child whose name appears on the Kiddy Plan application form sent to the Head Office of Industrial Alliance Insurance and Financial Services Inc., hereinafter called the “Company”, must permanently reside in Canada during the coverage period.

A single critical illness insurance policy – children is issued for all eligible children of a same family, who are insured under a single contract.

Lifetime limit of a single free critical illness insurance coverage per child.


Benefit

Benefit paid on the diagnosis of critical illness: $2,000. When more than one child is insured under this policy, the benefit is payable once, in a single payment to the contractholder, on the first diagnosis of a critical illness.

Conditions of Payment

The benefit is only available if it is the first occurrence of a critical illness. The diagnosis of a critical illness must be made by a medical specialist, authorized to practice in Canada, in accordance with the covered medical conditions of this policy.

If the diagnostic techniques for a critical illness are not indicated in the description of the said critical illness in the Covered Medical Conditions section of this policy, the diagnosis must be made by a medical specialist, authorized to practice in Canada and confirmed by modern and usually used investigative techniques relating to this illness at the time settlement is requested.

Extent of Protection

The insurance takes effect on the date the application form is received at the Head Office of the Company. The period of coverage is 12 months.

Termination of Coverage

The coverage for all children insured under this policy terminates when the first of the following events occurs:


On the date the critical illness benefit is paid to the contractholder after one of the insured children is diagnosed with a critical illness; or


Twelve months after the date this policy takes effect.

Diagnosis Outside of Canada

If a critical illness is diagnosed outside of Canada by a medical doctor in a jurisdiction deemed acceptable by the Company, the benefit will be payable only if all of the following conditions are satisfied:


The complete medical records are made available to the Company;


Based on these medical records, the Company is satisfied that:


The same diagnosis would have been prescribed if the illness had been diagnosed by a medical doctor practising in Canada;


The same treatment would have been prescribed under Canadian standards;


The same treatment, involving the particular surgical procedure, if any, would have been recommended if treatment had been administered in Canada.

The Company may require that the insured undergo an independant medical examination by a medical doctor appointed by the Company. In the case of elective surgery, such examination must be undergone before surgery is performed.

Definitions

Child: means a child of the contractholder between 2 and 5 years of age inclusively, including a child legally adopted by the contractholder or the contractholder’s spouse, who is named in the Kiddy Plan application form and who is eligible upon enrolment in the plan.

Children of a same family: means all children of the contractholder.

Contractholder: means the parent or tutor whose name appears on the application form and whose child or children are eligible for the program.

Critical illness: means one of the conditions described in the Covered Medical Conditions section, which is diagnosed while the coverage is in force and for which the insured child for whom the diagnosis was made lives for at least 30 days after the date of the diagnosis or during the specific period of the diagnosed condition.
